IMPORTANT INFO: If your child is taking lessons at a high school, please indicate which in the “current school” field. Voice lessons are billed at the time of the lesson and due on the last day of the month. Due to processing costs, there will be a 3% fee added to all credit/debit card transactions, whereas there is no fee on bank account transactions. Though account information is collected now, there will be no charges until lessons occur. There will be no late fees, but I will be unable to see students with an outstanding balance and will be proactive about resolving payment before a missed lesson. After filling out this form, you will receive login information for My Music Staff - please create your account promptly so that you can monitor your voice lesson experience entirely through that website, including enrolling in autopay (if possible) to be sure that balances stay current.
